Moreover, rosacea can produce other discomforts such as a burning or stinging sensation.

In a split-face trial comparing microsecond 1064-nm neodymium:yttrium-aluminum-garnet (Nd:YAG) laser with nonpurpuragenic 595-nm pulsed dye laser (PDL) for persistent facial redness associated with rosacea, the study found that both lasers effectively treated the symptom with no serious adverse events observed. [1] Fourteen patients received 4 treatments each at one-month intervals with PDL or Nd:YAG in both cheeks. Their skin color and characteristics, digital photographs, pain scores, and preferences were then recorded. Results showed a greater reduction in facial redness with PDL compared to Nd:YAG.

 

A Phase 4 multicenter study assessed the safety and tolerability of daily oxymetazoline in addition to various energy-based therapies for moderate to severe facial redness related to rosacea. [2] The energy-based therapies included the Excel® V (Cutera, Brisbane, CA) and other laser systems. A total of 46 patients with rosacea participated in the study. Results showed that treatment with oxymetazoline in addition to energy-based therapies such as Excel® V reduced facial redness associated with rosacea.

 

A study, conducted through October 13, 2021, involved an analysis of several studies from large online databases. [3] Ten articles were included in this study. The results indicated that both PDL and Nd:YAG laser treatments were associated with over 50% clearance improvements and high patient satisfaction rates. In addition, the pain scores for both treatments did not significantly differ. This suggests that both treatments have similar efficacy and pain tolerability in treating rosacea.

Sunspots, also known as age spots or liver spots, present as dark, pigmented areas and commonly result from prolonged sun exposure. On the other hand, hyperpigmentation is characterized by uneven skin color due to the excess production of the skin’s natural pigment called melanin. Sunspots and hyperpigmentation are usually harmless but they can cause low self-esteem in some individuals.

 

Both Laser Genesis and Excel V+ can reduce the appearance of sunspots and hyperpigmentation by breaking down the skin pigment called melanin. They work by delivering precise wavelengths of light or laser energy into the target area/s, which in turn is absorbed by the melanin. As melanin breaks down, the pigment is naturally eliminated by the body. Over time, new, healthier skin cells replace the treated skin, resulting in a more even appearance.

 

Seventeen females with darkening of the skin in the axillary (underarm) area as a result of inflammation or injury received low-fluence Q-switched 1064-nm Nd:YAG laser treatment every 2 weeks. [4] After 3 treatment sessions, a significant improvement in the appearance of the underarm skin was observed. This result lasted for at least 6 months after the last session.

 

A study assessed the effectiveness and safety of the 1,064 nm Q-switched Nd:YAG laser in treating patients with melanocytic nevi (commonly known as moles). [5] Over 2,000 patients were included in the study and received the treatment. After one treatment session, 70% of the moles were completely removed while the other 30% required three sessions.

 

A study evaluated the effect of the combination of 1,064-nm QS-Nd:YAG laser and ultrasonic application of topical vitamin C in patients with melasma (brown or gray-brown patches on the facial skin). [6] Eight patients with melasma received four laser sessions. During the second to fourth sessions, the appearance of melasma in these patients was significantly reduced without any adverse effects.

A study compared the effectiveness and safety of Grid RF and Laser Genesis for treating periorbital rhytides (wrinkles around the eyes). [7] It involved eight patients and found no statistically significant difference in improvement between the two treatments after two months. Both treatments were safe and well-tolerated by the patients, with no adverse effects observed during the follow-up period.

 

In one study, 51 patients with various skin types were treated with the Nd:YAG laser for skin rejuvenation. [8] Out of these, 34 patients completed at least 7 treatments, and the results showed subtle improvements in wrinkles and skin laxity. Both patient self-assessments and physician evaluations indicated positive changes. The 1064-nm Nd:YAG laser was well tolerated by patients with different skin types, and no adverse events were reported.

 

In a study involving thirty females with Fitzpatrick skin type IV, the 1064 nm Nd:YAG laser was used in a dual toning technique. [9] This involved two different laser modes with multiple passes until maximum improvement was achieved. Results showed that 67% of patients reported fair to excellent improvement, 23% had visible improvement, and 10% had little or no improvement. No unexpected side effects were observed, and the technique was well-tolerated without anesthesia.

Spider Veins and Vascular Lesions

Spider veins and vascular lesions refer to the abnormal blood vessels that are visible on the surface of the skin. They usually appear as purple lines or patches. While these skin conditions are usually harmless, they can cause physical discomforts, such as aching, burning, or itching, as well as low self-esteem.

 

Both Laser Genesis and Excel V+ can target abnormal blood vessels on the surface of the skin.  By delivering laser energy into the skin, the abnormal blood vessels gradually close off and collapse. Over time, the treated blood vessels are naturally eliminated by the body, resulting in smoother and healthier-looking skin.

 

In a study involving 15 female subjects seeking treatment for leg veins and spider veins, a protocol with two diode laser treatment sessions six weeks apart showed improvements. [10] The average vascular lesions classification reduced from a baseline score of 2.9 (moderate) to an average score of 2 (mild) at the 3-month follow-up, with this improvement sustained at the 6-month follow-up. The treatment was found to be safe and effective for leg vascular lesions on the lower extremities, with no significant adverse events observed.

 

A study assessed the effectiveness and safety of the long-pulsed 1064 nm Nd:YAG laser in treating superficial cutaneous vascular lesions. [11] Over three years, 255 patients were treated, primarily for spider angiomas, facial telangiectasia (enlargement of small blood vessels), and leg telangiectasia. Results showed that 100% of spider angiomas, 97% of facial telangiectasia, and 80.8% of leg telangiectasia significantly improved or cleared with this laser treatment.

 

A millisecond contact-cooled 1064 nm Nd:YAG laser was evaluated for treating telangiectases and small reticular veins on the lower extremities. [12] Twenty-one sites with various skin types received two laser treatments, resulting in significant improvement in 71% of treated vessels of all colors and sizes.

Over time, the treated blood vessels are naturally absorbed by the body, making them less visible.

 

A comparison study was conducted to assess the efficacy and side effects of 532-nm KTP and 1064-nm Nd:YAG lasers for treating cherry angiomas (small, benign, red or purple skin growths composed of blood vessels). [13] Results showed that the KTP laser required significantly more sessions for complete clearance but had a lower risk of side effects. In contrast, the Nd:YAG laser was more efficient in terms of sessions.

 

A 59-year-old female patient with a large venous-type hemangioma (a benign, non-cancerous growth of blood vessels) in the muscle of her left leg was successfully treated using Nd:YAG laser therapy in two sessions. [14] The treatment resulted in a substantial reduction in the lesion size, estimated at a 75%-80% reduction at a 6-month follow-up after the second treatment. No permanent adverse effects were observed.

 

A study conducted at a tertiary care hospital involving 29 patients with vascular lesions treated using long-pulsed 1064 nm Nd:YAG laser found promising results. [15] After six months of treatment, 46.15% of patients achieved complete healing, while 53.84% showed good improvement in their lesions. No permanent side effects were observed.

However, the application of a topical carbon solution did not enhance the laser’s efficacy. Overall, QSNYL was considered a safe and effective skin rejuvenation method for Asian patients.

 

A study assessed the effectiveness of Q-switched 1064-nm Nd:YAG laser treatment for keratosis pilaris (KP), a skin disorder characterized by small, rough bumps on the skin. [17] Twelve patients underwent ten laser sessions with positive results, with over 50% improvement in skin texture observed in half of the patients and 41.7% improvement in skin discoloration.

 

A study included 35 female patients with hand skin photoaging (premature aging of the skin on the hands due to sun exposure). [18] They underwent a combination treatment of IPL and Nd:YAG laser on one hand and Nd:YAG laser alone on the other hand. The combination treatment resulted in more significant improvements in pigment distribution, fine wrinkles, sallowness, pigment tone, and overall scores compared to Nd:YAG laser treatment alone, indicating its superiority in rejuvenating photoaged hand skin.
